In some regions of the world obesity has assumed epidemic proportions, and in particular the incidence of diabetes mellitus as one of the most important obesity associated diseases is doubling every 15 years. The first law of thermodynamics is the thermodynamic expression of the principle of the conservation of energy. So when energy is added to a system, it is either stored or used to perform work. Thus, obesity results from an imbalance between energy intake and energy expenditure. So either energy intake or energy expenditure could influence to achieve the ideal fat mass. Most anti-obesity interventions – behavioural or pharmacological – target food intake and are limited effective. One notable exception is bariatric surgery – with most pronounced weight loss within the first years which – ultimately – also leads to reduced morbidity and mortality. On the other hand interventions that target at energy expenditure are limited. Physical activity is effective when performed as a long term intervention. However, physical activity is sometimes hard to persevere.
